ID Badge Request Form
Complete and submit this form to request an ID Badge for individual(s) other than the custodian parents/legal guardians who will pick up a student daily or frequently (examples include: nannies, caregivers, Grandparents, etc.)
Temporary badges for occasional visitors or care providers are available at the front reception desk.
IMPORTANT INFORMATION: By requesting an ID badge you are authorizing this person or persons to be added to your student’s Emergency Contact Form.
Note: Anyone picking up or dropping off a student must be listed on the student’s Emergency Contact Form.
